_You are hereby notified that your earnings, up to 25% of your nonexempt earnings
20 are being garnished in payment of your indebtedness to the United States, in CV-
00-1219-PHX—RCB, District ofArizona, in the sum of$7,934.24. A balance of$9,200.20
21 remains outstanding on this judgment.
22 You are also notified that there are exemptions under the law which protect
approximatelly 75% of your earnings. Your earnings are exempt except for the lesser of
23 25% per wee of your wages or the amount in excess of 30 times the minimum wage._ If
you have any orders of alimony or child support algainst you, or if you have other prior
24 garmshments against you, they take priority over t is garnishment. If you think you are
not being given the benefit of the above exemption, or other court orders or garnishments
25 are not berng given proper priority, you may ask the court for a hearing.

_ The hearing will take place within 10 days after the Clerk receives your request,

4 At the hearing you may explain to the judge why you think you do not owe the
money to the Government. If you o not request a hearing within 20 days of receiving this
5 notice, approximately 25% will be withheld from your earnings and paid to the
6 Government to be applied to your debt.
If you think you live outside the Federal judicial district in which the court is
7 located,_you may request, not later than 20 days after you receive this notice, that this
proceeding to take your earnings be transferred by the court to the Federal judicial district
8 in_wh1ch you reside. You must make your reepuest in writing, and either mail it or deliver
