Qwest Communications Settlement Fund,"for the amount of $595,233.95 payable to Gilardi & Co., LLC, for the payment of expenses incurred, as provided in the Declaration. The funds shall be distributed from the earnings of the Distribution Fund. The check shall contain the notation " SEC v. Qwest Communications Settlement Fund,"the Federal tax identification number for the Qualified Settlement Fund 20-2883185, and payment identification " Distribution Agent expenses."
