DECLARATION OF JUDE P. DAMASCO

I, Jude P. Damasco, am over eighteen years of age and state the following facts which I know of my own personal knowledge. 1. I am the managing partner of Damasco & Associates LLP ("Damasco"), which is

located at 505 Sansome Street, Suite 1701, San Francisco, CA 94111. I am a certified public accountant. 2. Damasco was appointed as Tax Administrator for the SEC v Qwest Communications

Settlement Fund in the Order Appointing Tax Administrator, Civil Action No. 04-D-1267 (MJW) filed on June 8, 2005. 3. As Tax Administrator, Damasco has determined that the SEC v Qwest

Communications owes $726,500 in estimated tax liability for the first quarter of 2006. This tax payment is due April 17, 2006. 4. Please authorize the transfer of tax liability via wire to Damasco by Gilardi & Co.,

LLC, Distribution Agent as appointed in the order filed February 28, 2006. 5. The funds should be received in Damasco's Trust account on or before April 12, 2006.

The tax payment will be made once the funds are received. Pursuant to 28 U.S.C. § 1746, I declare under penalty of perjury that the foregoing is true and correct. Executed on March 22, 2006 in San Francisco, California.
