DECLARATION OF JUDE P. DAMASCO I, Jude P. Damasco, am over eighteen years of age and state the following facts which I know of my own personal knowledge. 1. I am the managing partner of Damasco & Associates LLP ("Damasco"), which is

located at 505 Sansome Street, Suite 1701, San Francisco, CA 94111. I am a certified public accountant. 2. Damasco was appointed as Tax Administrator for the SEC v. Qwest Communications

Settlement Fund in the Order Appointing Tax Administrator, Civil Action No. 04-D1267(MJW) filed June 8, 2005. 3. As Tax Administrator, Damasco has determined that the SEC v Qwest

Communications Settlement Fund owes $50,691 in tax liability for the year 2005. This tax payment is due March 15, 2006. 4. A check should be made payable in the amount of $50,691 to "Damasco & Associates,

Trust Account" and sent to Damasco & Associates LLP, 505 Sansome Street, Suite 1701, San Francisco, CA 94111. For timely payment, it should be received in our offices on or before March 8, 2006. 5. The check will be deposited into a trust account from which the tax payments will be

made. Therefore, the Employer Identification Number ("EIN") that should be written on the check is 20-2883185 because it is for tax payment on behalf of SEC v Qwest Communications Settlement Fund.
