The Distribution Agent shall issue a check on Bank of the West account number 756-013215, under the case name designation " SEC v. Qwest Communications Settlement Fund,"for the amount of $50,691 payable to Damasco & Associates, for the payment of tax obligations, as provided in the Declarations. The funds shall be distributed from the earnings of the Distribution Fund. The check shall contain the notation " SEC v. Qwest Communications Settlement Fund,"the Federal tax identification number for the Qualified Settlement Fund 20-2883185, and payment identification " 2005 tax return."
