IN THE UNITED STATES DISTRICT COURT FOR THE DISTRICT OF ARIZONA BARBARA BOOTH, Plaintiff, v. QWEST COMMUNICATIONS INTERNATIONAL, INC., a Foreign Corporation; and SEDGWICK CLAIMS MANAGEMENT SERVICES, INC., a Foreign Corporation, Defendants. On July 15, 2005, this Court issued an Order granting the Motion for Summary Judgment in favor of the Defendants in this matter. On July 18, 2005, judgment was entered dismissing the above-referenced matter with prejudice. Since Entry of Judgment, the Plaintiff and the undersigned have developed fundamental disagreements as to the future action which may or could be taken regarding this case. These disagreements have become so profound that it is highly unlikely that the two will be able to continue to work effectively with each other. Additionally, the client has failed substantially to fulfill an obligation to counsel regarding counsel's services. Counsel and the firm request withdrawal as soon as possible. Counsel advises the Court that Plaintiff has been notified in writing of the status of the case, including the dates and times for compliance with the rules of court regarding appeal of this matter. ... ...
