l Petition to Revoke Supervised Release
COMES NQW PROBATION OFFICER Myra Roper presenting an official report on Alexander Lee
THOMAf$ who was congrnitted to the Bureau of Prisons on August 10, 2004, by the Honorable David G.
Campbell presiding in the District Court ofArizona. A 3-year period of supervised release was imposed,
and supervision commenced upon the offender's discharge from imprisonment on August 26, 2005. ln
addition to the general terms and conditions adopted by the Court in General Order 05-36, the offender
was ordered to comply with the following special conditions:
1. You shall participate as instructed by the probation officer in a program of substance abuse
treatment which may include testing for substance abuse. You shali contribute to the cost of
treatment in an amount to be determined by the probation officer.
2. You shall submit to search of person, property, vehicles, business, and residence to be conducted
in a reasonable manner and at a reasonable time by, or at the direction of, the probation officer.
3. You are prohibited from owning, maintaining or using a firearm. _
Alexander Lee THOMAS was convicted of Possession of a Firearm by a Convicted Felon.
RESPECTFULLY PRESENTING PETITION FOR ACTION OF COURT FOR CAUSE
AS FOLLOWS:
A) Violation of standard condition no. 8: "You shall refrain from excessive us of alcohol and
shall not purchase, possess, use, distribute or administer any narcotic or other controlled
substance, or any paraphernalia related to such substances, except as prescribed bya physician.
lf ordered by the Court to participate in a drug andlor alcohol abuse treatment program, you shall
totally abstain from the use of any alcoholic beverages or other intoxicants during and after the
course of your treatment."
On or about October 3, October 18 and October 26, Thomas used marijuana as evidenced by positive
urine tests submitted on those dates for THC. On or about November 1, 2005, Thomas used
amphetamines, methamphetamine, and Ti-IC as evidenced by a positive urine test submitted that date.
On or about December 23, 2005, Thomas used PCP as evidenced by a positive urine test submitted that
date. A Grade C violation.
B) Violation of special condition no. 1: "You shall participate as instructed bythe probation officer
in a program of substance abuse treatment which may include testing for substance abuse."
Thomas failed to submit urine tests as required on November 8, 22, and 30,2005; December 28, 2005;
and January 9, 2006, as required. A Grade C violation.

In conformance with the provision of 28 U.S.C. § 1746, i declare, under penalty of perjury, that the
foregoing is true and correct to the best of my knowledge. Based on the information presented that the

I find there is probable cause to believe the offender has violated conditions of supervision, supported
