Comp P-05 INSTRUCTION NO. _____ Personal injury protection benefits for any period are overdue if not paid within thirty days after the insurer receives reasonable proof of the fact and amount of expenses incurred. In the usual case, for the purposes of triggering the 30-day time period for payment of benefits, the following documents are sufficient to establish reasonable proof of the fact and amount of the expense: an application for benefits from the claimant and a billing statement for the procedure or treatment which contains the name and address of the treating health care provider, the evaluation or diagnosis and the medical procedure performed or the medical treatment provided, and an itemized statement of charges corresponding to the medical service or treatment provided along with corresponding dates of services.
