Please be advised that my patient, Mr. Larry Carr has completed 23 Intensity Modulated
Radiotherapy treatments followed by Palladium Seed implantation for his prostate
cancer. At the time of his seed implantation procedure it was decided that he would need
to have 8 additional radiation treatments. He will return to The Dattoli Cancer Center on
l Thursday, March 30, 2006 for a Post Treatment Simulation, on Monday, April 3, 2006 he
will have X-Rays. Mr. Carr will begin his first day of treatment on Tuesday, April 4,
2006 and should be completed by Thursday, April 13, 2006. Please allow 3 buffer days ‘
for any down time the treatment machine may experience,"total number of days expected
