Defendants submit the attached Proposed Jury Questionnaire. Counsel for Defendants forwarded the attached questionnaire to Plaintiffs' counsel on or about July 11, 2006 for his review and comment. Despite repeated requests for input, counsel

undersigned received no communication from Plaintiffs' counsel until 4:31 p.m. on August 9, 2006. Because undersigned counsel was out of the office on August 10, 2006 she did not have an opportunity to review Plaintiffs' proposed questionnaire until 1:00 p.m. on August 11, 2006. Because of Plaintiffs' counsel's lack of cooperation, there was insufficient time to submit a joint proposed questionnaire. Defendants, therefore, submit this separate Proposed Jury Questionnaire.
