On multiple occasions, l have requested that you provide me with signed
reieases _ To date. you hayte never sent me a signed
retease . You may believe that t obtained your MGH records because you signed ·
a release. This is incorrect. l obtained your records because t am legai counsel
for MGH and you are suing MGH.
lf you signed a release for Advanced Cardiac Specialists, you never
provided it to me or else it was lost in the mail. in any event, l never received
one. Therefore, please sign the enclosed release for ACS records and return to
me.
in addition, please sign the enclosed release for Hemet Globai Urgent
Care records and return to me as soon as possible.
l am unable to obtain records from the Urgent Care on Layton Street in
San Jacinto because you have not given me an addrees or phone number for
that facility. Please sign the enclosed release, return to me, and provide me with
more specific contact information for that facility.
