Petition to Revoke Supervised Release U ‘ AED
COMES NOW PROBATION OFFICER Matt L. Bass presenting an official report on Robert
DUBOS who was originally placed on probation August 13, 2002. He was continued on supervision April
13, 2003. On July 2, 2003, Dubos' probation was revoked and he was committed to the Bureau of
Prisons for11 months by the Honorable Earl H. Carroll presiding in the District Court of Arizona after
violating his terms of probation. A three period of supervised release was imposed, and supervision
commenced upon the offender's discharge from imprisonment on November 21, 2003.
On June 14, 2004, Dubos' supervised release was revoked and he was committed to the Bureau of
Prisons for 11 months by Your Honor after violating his terms of supervised release. A 25 month period
of supervised release was imposed, and supervision commenced upon the offenders discharge from
imprisonment on April 25, 2005. ln addition to the general terms and conditions adopted by the court in
General Order 99-9, the offender was ordered to comply with the followingxspecial conditions:
1. You shall participate as instructed by the probation officer in a program of substance abuse
treatment which may include testing for substance abuse. You shall contribute to the cost of
treatment in an amount to be determined by the probation officer.
2. You shall submit to search of person, property, vehicles, business, and residence, to be
conducted in a reasonable manner and at a reasonable time by, or at the direction of, the
probation officer.
3. You shall participate in a mental health program as directed by the probation officer which may
include taking prescribed medication. You shall contribute to the cost of treatment in an amount
to be determined by the probation officer.
4. You are prohibited from owning, maintaining or using a tirearm.
5. You shall abstain from all use of alcohol or alcoholic beverages.
6. You shall seek and obtain veritiable employment.
Robert DUBOS was convicted of Possession of Stolen Mail, a class D felony.

RESPECTFULLY PRESENTING PETITION FOR ACTION OF COURT FOR CAUSE
AS FOLLOWS: ,
A. Violation of Standard Condition No. 3: "You shall report to the Probation Office as directed by
the probation ofdcer and shall submit a truthful and complete written report within the first dye (5)
days of each month." Grade C violation.
1. Dubos failed to submit monthly supervision reports for June, August, September, October,
and November, 2005.
2. Dubos failed to appear in the U.S. Probation Office on November 3, 2005, as directed by
telephone on November 2, 2005.
B. Violation of Standard Condition No. 7: "You shall notify the probation ofhcer at least ten days
prior to any change of residence or employment. ” Grade C violation.
Dubos failed to notify this officer at Ieastten days priorto a change in residence. On October 31, 2005,
Dubos' grandmother advised this officer Dubos moved from her residence. This officer also spoke with
Dubos by telephone the same day and he also admitted to changing residences without proper
notification.
C. Violation of Special Condition No. 1: "You shall participate as instructed by the probation officer
in a program of substance abuse treatment which may include testing for substance abuse. You
shall contribute to the cost of treatment in an amount to be determined by the probation officer."
Grade C violation.
1. Dubos failed to participate in a substance abuse assessment at Tasc as directed in person on
July 22, 2005.
2. Dubos failed to submit to urinalysis on September 12, 22, October 10, 28, and November 1 , 2005.
3. Dubos used marijuana as evidenced by a rapid drug screen conducted by Recovery Homes on
June 27, 2005. During an office visit on July 22, 2005, Dubos admitted to this officer he used
marijuana prior to the drug screen.
D. Violation of Special Condition No. 3: "You shall participate in a mental health program as
directed by the probation officer which may include taking prescribed medication. You shall
contribute to the cost of treatment in an amount to be determined bythe probation ofdceiz "Grade
C violation.
Dubos failed to participate in a mental health assessment at Tasc as directed in person on July 22, 2005.

In conformance with the provision of 28 U.S.C. § 1746, I declare, under penalty of perjury, that the
foregoing is true and correct to the best of my knowledge. Based on the information presented that the
offender has violated conditions of supervision, I am petitioning the Court to issue a warrant.

ORDER OF COURT
I find there is probable cause to believe the offender has violated conditions of supervision, supported
by the above affirmation given under penalty of erjury. The Court orders the issuance of a warrant.
