3 Tim Jacobson ("Jacobson").
4 3. On September 2,2003, that Judgment was recorded in the records of

Maricopa

5 County, Docket No. 2003-1224296.

6 4. The last known address of the judgment debtor is: 8431 E. Wilshire Drive,
7 Scottsdale, Arzona 85257.

8 '5. The total amount owed by the judgment debtor on the date the Judgment was

9 granted was $7,986.62, plus post-judgment interest at the rate of 1 .29% per annum until paid.
