Petition to Revoke Supervised Release T"
COMES NOW PROBATION OFFICER Sharon D. Werner presenting an official report on Shelley
Miguel who was committed to the Bureau of Prisons on September 22, 2003, by the Honorable Susan
R. Bolton presiding in the District Court of Arizona. A three year period of supervised release was
imposed, and supervision commenced upon the offender's discharge from imprisonment on December
30, 2004. MigueI's supervised release was revoked on April 19, 2005, and she was committed to the
custody of the Bureau of Prisons for six months. A 27 month period of supervised release was imposed,
and supervision commenced upon the offender's discharge from imprisonment on September 2, 2005.
In addition to the general terms and conditions adopted by the court in General Order 04-1 1 , the offender
was ordered to comply with the following special conditions:
1. You shall participate as instructed by the probation officer in a program of substance abuse
treatment which may include testing for substance abuse. You shall contribute to the cost of
treatment in an amount to be determined by the probation officer.
2. You shall submit your person, property (including but not limited to computer, electronic devices,
and storage media), residence, office, or vehicle to a search conducted by a probation officer, at
a reasonable time and in a reasonable manner.
3. You shall participate in a mental health program as directed by the probation officer which may
include taking prescribed medication. You shall contribute to the cost of treatment in an amount
to be determined by the probation officer.
4. You shall reside and participate in a community corrections center for up to 180 days, unless
discharged earlier by the probation officer.
5. You are prohibited from owning, maintaining or using a firearm.
6. You shall abstain from all use of alcohol or alcoholic beverages.
7. You shall cooperate in the collection of DNA as directed by the probation officer.
8. You shall not reside at 325 Mullberry Street, Sacaton, Arizona 85247, unless approval is granted
by the supervising probation officer.
Shelley Miguel was convicted of CIR assault with a dangerous weapon.

RESPECTFULLY PRESENTING PETITION FOR ACTION OF COURT FOR CAUSE
AS FOLLOWS:
A. Violation of Special Condition N0. 4: "You shall reside and participate in a community
corrections center for up to 180 days, unless discharged earlier by the probation officer."
The offender failed to reside and participate in a community corrections centerfor up to 180 days.
Miguel was placed at Behavioral Systems Southwest (BSSW) in Florence, Arizona, on September
2, 2005. On September 16, 2005, Miguel checked out ofthe facility forjob search and she failed
to return. Miguel has not contacted BSSW or the undersigned and her where-a-bouts are
unknown.
Grade C violation. §7B1.1(a)(3)(B).
