Petition to Revoke Supervised Release T ‘
COMES NOW PROBATION OFFICER Adeline Bustamante presenting an official report on Jason
Ortega who was committed to the Bureau of Prisons on October 28, 2005, by the Honorable Mary H.
Murguia presiding in the District Court of Arizona. A 50 month period of supervised release was
imposed, and supervision commenced upon the offender‘s discharge from imprisonment on March 8,
2006. ln addition to the general terms and conditions adopted by the court, the offender was ordered to
comply with the following special conditions:
1. You shall participate as instructed by the probation officer in a program of substance abuse
treatment which.may include testing for substance abuse. You shall contribute to the cost of
treatment in an amount to be determined by the probation officer.
2. You shall submit your person, property (including but not limited to computer, electronic devices,
1 and storage media), residence, office, or vehicle to a search conducted by a probation officer, at
a reasonable time and in a reasonable manner.
3. You shall participate in a mental health program as directed by the probation officer which may
include taking prescribed medication. You shall contribute to the cost of treatment in an amount
to be determined by the probation officer.
4. You shall reside and participate in a residential re-entry center for 180 days, unless discharged
earlier by the probation officer.
· .5. You are prohibited from owning, maintaining or using a firearm.
6. You shall cooperate in the collection of DNA as directed by the probation officer.
Jason Ortega was convicted of Importation of Methamphetamine, a violation of21 U.S.C. §952 and 960, A
a Class A felony. Ortega was sentenced in the Southern District of California to 41 months imprisonment
to be followed by four years supervised release. Supervision commenced on April 12, 2002.
Jurisdiction was transferred to the District of Arizona on January 31, 2003, and assigned to the
Honorable Mary H. Murguia.
On March 16, 2005, the Court revoked Ortega's term of supervised release and sentenced him to four
months imprisonment, to be followed by 44 months supervised release.

RESPECTFULLY PRESENTING PETITION FOR ACTION OF COURT FOR CAUSE
AS FOLLOWS:
A. Violation of Special Condition #1: "You shall participate as instructed by the probation officer
in a program of substance abuse treatment which may include testing for substance abuse. You
shall contribute to the cost of treatment in an amount to be determined by the probation officer."
Ortega failed to submit to random urine testing on the following dates: September 6, September 18,
October 6, October 9, October 18, November 7, November 15, November 21, November 28, and
December 4, 2006. Grade C violation.
B. Violation of Special Condition #3: "You shall participate in a mental health program as directed
by the probation officer which may include taking prescribed medication. You shall contribute
to the cost of treatment in an amount to be determined by the probation officer."
On November 14, 2006, Ortega failed to keep his counseling appointment at Southwest Behavioral.
T Grade C violation. -
C. Violation of Special Condition #3: "You shall participate in a mental health program as directed
by the probation officer which may include taking prescribed medication. You shall contribute
to the cost of treatment in an amount to be determined by the probation officer."
On October 30, 2006, Ortega was prescribed Etavil, to help him deal with depression and pain; and
Abilify, to help him deal with paranoia and confusion. On November 29, 2006, Ortega failed to keep his
medication monitoring appointment with Southwest Behavioral. Grade C violation.

In conformance with the provision of 28 U.S.C. § 1746, I declare, under penalty of perjury, that the
foregoing is true and correct to the best of my knowledge. Based on the information presented that the
offender has violated conditions of supervision, I am petitioning the Court to issue a warrant.
