COMES NOW PROBATION OFFICER Darin Myler presenting an official report on STEVEN DIRK
SANTOS, Jr. who was convicted of assault with a dangerous weapon and was committed to the Bureau
of Prisons on June 9,2004, by the Honorable Susan R. Bolton presiding in the District Court of Arizona.
A 36 month period of supervised release was imposed, and supervision commenced upon the offender's
discharge from imprisonment on February 17, 2006. tn addition to the general terms and conditions
adopted by the court in General Order 05-36, the offender was ordered to comply with the following
special conditions:
1. You shall participate as instructed by the probation officer in a program of substance abuse
treatment which may include testing for substance abuse. You shall contribute to the cost of
treatment in an amount to be determined by the probation officer.
2. You shall submit to search of person, property, vehicles, business, and residence to be conducted
in a reasonable manner and at a reasonable time by, or at the direction of, the probation officer.
3. You shall participate in a mental health program as directed by the probation officer which may
include taking prescribed medication. You shall contribute to the cost oftreatment in an amount
to be determined by the probation officer.
4. You shall not contact with the victim, and the probation officer will verify compliance.
5. You shall abstain from all use of alcohol or alcoholic beverages.
6. You shall cooperate in the collection of DNA as directed by the probation officer.
RESPECTFULLY PRESENTING PETITION FOR ACTION OF COURT FOR CAUSE
AS FOLLOWS:
A. Violation of standard condition 7: You shall notify the probation officer at least ten days
. prior to any change of residence or employment.
On or about ll/larch 8, 2006, Santos relocated from his residence in Bapchule, Arizona. Santos
failed to make prior notice to his probation officer regarding his change in residence. Santos'
current whereabouts is unknown. A Grade C violation. §7B1.1(a)(3)(B). _

B. Violation of standard condition 9: You shall not purchase, possess, use, distribute or
administer any narcotic or other controlled substances as defined in section 102 of the
Controlled Substance Act (21 U.S.C. §801) or any paraphernalia rleated to such
substances, without a prescription by a licensed medical practitioner. Possession of
controlled substances will result in mandatory revocation of your term of supervision.
On or about February 22, 2006, Santos used marijuana as evidenced by a positive drug test and
his admission. Santos signed a written admission and stated he "smoked a bowl of marijuana."
A Grade C violation. §7B1.‘l(a)(3)(B).
C. Violation of special condition 5: You shall abstain from all use of alcohol or alcoholic
beverages.
On or about February 23, 2006, Santos used alcohol as evidenced by an alcohol test and his
admission. Santos told this officer he drank about four beers. A Grade C violation.
§7B1.1(a)(3)(B).

AFFIDAVIT AND PETITION PRAYING THAT THE COURT WILL ORDER A WARRANT BE ISSUED
In conformance with the provision of 28 U.S.C. § 1746, I declare, under penalty of perjury, that the
foregoing is true and correct to the best of my knowledge. Based on the information presented that the
offender has violated conditions of supervision, l petition the Court to issue a warrant.
