COMES NOW PROBATION OFFICER Leigh Anne Gibbons presenting an official report on CHARLOTTA KAYE LEWIS
who was committed to the Bureau of Prisons on May 17, 2004, by the Honorable RogerG. Strand presiding in the District Court
of Arizona. A 36-month period of supervised release was imposed, and supervision commenced upon the offenders discharge
from imprisonment on September 9, 2005. ln addition to the general terms and conditions adopted by the Court in General
Order 99-9, the offender was ordered to comply with the following special conditions:
1. lf deemed appropriate by the supervising probation officer, you shall participate as instructed by the probation officer
in a program of substance abuse treatment which may include testing for substance abuse. You shall contribute to the
cost of treatment in an amount to be determined by the probation officer.
2. You shall submit to search of personrpropeny, vehicles, business, and residence to be conducted in a reasonable
manner and at a reasonable time be, or at the direction of the probation officer.
3. You shall provide the probation officer access to any requested financial information.
4. You shall abstain from all use of alcohol or alcoholic beverages.
5. You shall complete your high school education as directed by the probation officer.
6. You shall not have contact with co-defendant Jalen Joe Joaquin without prior permission of the probation officer and
shall immediately report to the probation officer any unauthorized contact.
On February 21, 2006, Lewis signed a Waiver and Order for an unfavorable modification of conditions of supervised release
which added the following condition:
7. You shall reside and participate in a community corrections center for 90 days, unless discharged earlier by the
probation ofhcer.
CHARLOTTA KAYE LEWIS was convicted of aiding and abetting Transportation of Illegal Aliens for profit, a Class C felony in
violation of 8 USC §1324(a)(1)(A)(v)(I|) and (a](1)(B)(ii).
RESPECTFULLY PRESENTING PETITION FOR ACTION OF COURT FOR CAUSE AS FOLLOWS:
Allegation A: Violation of Standard Condition No. T: You shall notify the probation officer at feast ten days prior to any
change of residence or employment.
Lewis failed to report tothe probation officer of her change of residence. Grade C violation.

In conformance with the provision of 28 U .S.C. § 1748, I declare, under penalty of perjury, that the foregoing is true and correct
