COMES NOW PROBATION OFFICER Craig W. Haraga presenting an official report on Ignacio
Reyes-Perez who was placed on probation on April 25, 2005, by the Honorable Roger G. Strand
presiding in the District Court of Arizona. A three-year period of probation was imposed. In addition to
the general terms and conditions adopted by the court, the offender was ordered to comply with the
following special conditions:
1. If deported, you shall not re-enter the United States without legal authorization.
2. You shall provide the probation officer access to any requested financial information.
3. You shall participate in a Home Confinement Program without electronic monitoring under the
sanction of detention for a period ofthree months.
4. You shall cooperate inthe collection of DNA as directed by the probation officer.
Special condition added via waiver and order May 11, 2005:
5. You shall participate as instructed by the probation officer in a program of substance abuse
treatment which may include testing for substance abuse. You shall contribute to the cost of
treatment in an amount to be determined by the probation officer.
Ignacio Reyes-Perez was convicted of Conspiracy to Produce False Identification Documents, in violation
of 18 U.S.C. §371 and 18 U.S.C. § 1028(a), a Class D Felony.
RESPECTFULLY PRESENTING PETITION FOR ACTION OF COURT FOR CAUSE
AS FOLLOWS:
A. Violation of Standard Condition No. 9: "You shall not purchase, possess, use, distribute
or administer any narcotic or other controlled substance as defined in section 102 of the
Controlled Substances Act (21 U.S.C. § 801) or any paraphernalia related to such
substances, without a prescription by a licensed medical practitioner. Possession of
controlled substances will result in mandatory revocation of your term of supervision."
The offender used cocaine on or about April 26, 2005, as evidenced by a positive rapid drug
screen test which was confirmed by Ouest Diagnostics. In addition, the offender used cocaine
on or about It/lay 10, 2005, as evidenced by a positive drug test and signed admission by the
offender. A Grade C Violation. §7B1.1(a)(3).

B. Violation of Special Condition No. 5: "You shall participate as instructed by the probation
officer in a program of substance abuse treatment which may include testing for substance
abuse. You shall contribute to the cost of treatment in an amount to be determined by the
probation officer."
The offender failed to submit to substance abuse testing at TASC on August 23, 2006; August
26, 2006; September 13, 2006; September 22, 2006; October 16, 2006; October 25, 2006;
October 30, 2006; November 16, 2006, and November 27, 2006. A Grade C Violation.

In conformance with the provision of 28 U.S.C. § 1746, I declare, under penalty of perjury, that the
