2. 1 am the manager of Jays Twelve LLC ("Jays, LLC"), a general partner of
J l2ALH Associates (".l12ALH*’) and I have personal knowledge of the business activities
of J12ALH and Jays, LLC. I

4. The general partners of J 12ALH are Erica Jesselson and Jays Twelve _
LLC, a Delaware limited liability company.
5. J I.2ALH owns approximately 17% ofthe Class B membership interest in
