On above date and timeCIOBaker Glenenise and myself CIO Raney,Roger were locking bid 22 dwing inmates in from chow as I
CIO Raney proceeded to secure the lower cell doors on dwing I noticed IIIvI VVOIf,Jack SBl#00093532,waIk towards the teir
sliders on dwing I asked IIIVI Wolf were he was going IINI Wolf then stated in a very loud and angry outburst,“fuck you my back is I
broke I'm going to see a doctor,"I informed IIIVI Wolf that there wasnt any medical personell in the building IIM Wolf then stated "I I
aint fucking locking in until I see a doctor,"at that time IIIVI Wolf sat down on the teir at that time C/O Baker gave IIIIII WOLF a I
direct order to go lock -in lIl\Ii Wolf stated again fuck you I aint locking in we (CIO BAKER and CIO RANEY) placed handcuffs
on IIIVI Wolf and escorted him to bld 22s observation room to wait for the area LT to come and see him.

Description of Incident:
On above date and time CIO Carter,Ramon;LT Seacord,Thomas;and myself C/O Raney, Roger were in bid 22 obsenration room
with inmate Wolf,Jack SBI#00093532.IIl\Il WOLF had requested to see the area LT(LT SEACORD,LT BOONE STEPHEN was
unavailable at the time of incident) (incident#1014121) .l.T Seacord asked IIM Wolf what was wrong with him,IIM WOLF stated in
a very hostile tone "my fucking back is broken and l want to see a doctoi"LT Seacord stated to IIM WOLF "Sir you need to calm I
down so I can ask you what is goin on".!/M Wolf then stated" fuck you take these fucking handcuffs off of me my fucking back is I
brol wasn‘t anything wrong with IIM Wolf. IIM Wolf then stated "fuck you"and Eunged toward LT Seacord, thats when IIM Wolf was
restrained by myself(CIO RANEY and CIO CARTER and resecured in the observation room. Building 24's rovers were then
catled and dispatched to escort I/M Wolf to building 24 end of report.
