SUPPLEMENTAL JURY QUESTIONNAIRE COURT INTRODUCTION The purpose of this questionnaire is to provide information about your background and experience to assist the court and save time in jury selection. This questionnaire will not be used for any purpose other than to assist the court, the attorneys, and their clients with the jury selection process. Please answer each question below as completely and accurately as you reasonably can. Your complete written the answers will save a great deal of time for the judge, the lawyers, and you.
