COURT INTRODUCTION To JUROR QUESTIONNAIRE The purpose of this questionnaire is to provide information about your background and experience to assist the court and to save time in jury selection. The completed questionnaire is not a public document. This questionnaire will not be used for any purpose other than to assist the court, the attorneys, and their clients with the jury selection process. Please answer each question below as completely and accurately as you reasonably can. Your complete written answers will save a great deal of time for the judge, the lawyers, and you. If there is not sufficient space provided for you to complete your answers, please continue your answers on the last page, indicating the question number. Do no write on the reverse side of any page. Please write legibly and use a black pen. You are required to sign your questionnaire, and your answers will have the effect of statements given to the court under oath. Please note that at the end of the questionnaire, you will swear or affirm that your answers are truthful to the best of your knowledge. We need your very best and honest effort to answer all of the questions. Your full cooperation is of vital importance. If there are any questions that you would feel more comfortable answering in private, simply write "private" in the space provided for your answer. The follow-up questions on that topic will be held out of the presence of the other prospective jurors. Thank you in advance for your careful and honest response to the questionnaire.
