COMES NOW PROBATION OFFICER Michael A. Lipscomb presenting an official reporton John
David Johnson WHITE who was committed to the Bureau of Prisons on July 6, 2004, by the Honorable
Frederick J. Martone presiding inthe District Court of Arizona. A 36 month period of supervised release
was imposed, and supervision commenced upon the offenders discharge from imprisonment on August
20, 2004. In addition to the general terms and conditions adopted by the court, the offender was ordered
to comply with the following special conditions:
. 1. You shall submit to sea rch of person, property, vehicles, business, and residence to be conducted
l in a reasonable manner and at a reasonable time by, or at the directions of, the probation officer.
2. You shall provide the probation officer access to any requested financial information.
3. You are prohibited from making major purchases, incurring new financial obligations, or entering
into any financial contracts without the prior approval of the probation officer.
4. You shall participate in a Home Confinement Program for a period of five (5) months under the
sanction of detention with or without electronic monitoring at the discretion of the probation officer.
You shall contribute to the cost of electronic monitoring in an amount to be determined by the
probation officer.
5. . You are prohibited from owning, maintaining, or using a firearm. You shall not possess a firearm.
You are prohibited from selling or trading firearms in any way, including by way of gift or
otherwise.
John David Johnson WHITE was convicted of Dealing in Firearms Without a Federal License, in violation
of 18 USC §§ 922(a)(1)(A) and 924(a)(1)(D), a class D felony offense, as charged in Count 1 of the
superseding indictment.
RESPECTFULLY PRESENTING PETITION FOR ACTION OF COURT FOR CAUSE
AS FOLLOWS:
A: Violation of Standard Condition No. 1: You shall not commit another federal, state, or local
crime during the term of supervision.
White, on or about April 15, 2006, knowingly possessed a Romanian Arms, Model SAR-1, 7.62
x 39mm rifle, serial number S1-49358-2001; a Hi-Point, Model C9, 9mm handgun, serial number
P127968; 77 rounds of Wolf 7.62 x 39mm ammunition; and eight (6) rounds of Federal 9mm
ammunition, as reflected in Maricopa County Sheriffs Office departmental report number 06-
· 61030, in violation of 18 USC §§ 922(g)(1) and 924(a)(2). Each a Grade A Violation

AFFIDAVIT AND PETITION PRAYING THAT THE COURT WILL ORDER A WARRANT BE ISSUED
in conformance with the provision of 28 U.S.C. § 1746, I declare, under penalty of perjury, that the
foregoing is true and correct to the best of my knowledge. Based on the information presented that the
offender has violated conditions of supervision, I am petitioning the Court to issue a warrant.
