COMES NOW PROBATION OFFICER James M. Blount presenting an official report on Mark
Santo Demasse Ill who was committed to the Bureau of Prisons on July 29, 2004, by the Honorable
Susan R. Bolton presiding in the District Court of Arizona. Athree—year period of supervised release was
imposed, and supervision commenced upon the offender's discharge from imprisonment on May 26,
2005. In addition to the general terms and conditions adopted by the Court, the offender was ordered
to comply with the following special conditions:
1) You shall participate as instructed by the probation officer in a program of substance abuse
treatment which may include testing for substance abuse. You shall contribute to the cost of
treatment in an amount to be determined by the probation officer.
2) You shall submit to search of person, property, vehicles, business, and residence to be conducted
in a reasonable manner and at a reasonable time by, or at the direction of, the probation officer.
3) You shall participate in a mental health program as directed by the probation officer which may
include taking prescribed medication. You shall contribute to the cost of treatment in an amount
to be determined by the probation officer.
4) You shall complete 100 hours of community sen/ice at the rate of not less than 10 hours per
month as approved and directed by the probation officer, to commence within 30 days of the
beginning of supervision to be completed within the first year of supervision.
5) You are prohibited from owning, maintaining, or using a firearm.
6) You shall not be involved with gang activity, possess any gang paraphernalia, or associate with
any person affiliated with a gang.
7) You shall abstain from all use of alcohol or alcoholic beverages.

RESPECTFULLY PRESENTING PETITION FOR ACTION OF COURT FOR CAUSE
AS FOLLOWS:
A. Violation of Standard Condition 3: "You shall report to the Probation Ofiice as directed by the
Court or probation ofliceig and shaii submit a truthful and compiete written report within the tirst
tive days of each month. "
1) When supervision commenced in May 2005, the defendant was directed to report to
the probation office in person on the last Thursday of each month. The defendant has not
reported to the office since March 2006.
2) The defendant has not submitted a written monthly status report since March 2006.
B. Violation of Special Condition 1: "You shalt participate as instructed by the probation ohicer in
a program of substance abuse treatment which may inciude testing for substance abuse. You
shall contribute to the cost of treatment in an amount to be determined by the probation ofiicer. "
The defendant is required to submit to substance abuse testing at TASC and he is currently
assigned the color Tan. In short, the defendant is required to submit two drug tests per month and
he has missed the following dates: April 11, 28, May 9, 31, June 8, and 27, 2006. Additionally,
documentation verifying the missed drug tests are appended to the petition.
**AII of the above-indicated allegations are Grade C violations per §7B1 .‘i(a)(3)(B) and Allegation B calls
for mandatory revocation of supervision per 18 USC § 3583(g)(3).
AFFIDAVIT AND PETITION PRAYING THAT THE COURT WILL ORDER A WARRANT BE ISSUED
ln conformance with the provision of 28 U.S.C. § 1746, I declare, under penalty of perjury, that the
foregoing is true and correct to the best of my knowledge. Based on the information presented that the
offender has vi lated conditions of supervision, l am petitioning the Court to issue a warrant.
