Upon release from custody, the defendant is placed on supervised release for a term of THIRTY-SIX (36) MONTHS less credit for time served, under the standard conditions adopted by this Court in

General Order 05-36, and the following special conditions: 1. You shall participate as instructed by the probation officer in a program of substance abuse treatment, which may include testing for substance abuse, and contribute to the cost of treatment in an amount to be determined by the probation officer. 2. You shall provide the probation officer access to any requested financial information. 3. You shall seek and maintain full time employment, unless enrolled in a full time educational program. 4. You shall abstain from all use of alcohol or alcoholic beverages. 5. Upon release from custody you shall reside and participate in a Community Correction Center or halfway house for up to 90 days unless discharged earlier by the probation officer on a work release program. And comply with the conditions of the facility

where you're placed. 6. During the period of supervised release you shall not commit another federal, state or local crime, and that you shall report to the U.S. Probation office within 72 hours of release from custody. 7. You shall abstain from the use of illicit substances. IT IS FURTHER ORDERED that the probation office report any violation of the defendant with respect to his conditions of supervised are release, and that a petition be filed for violation with a warrant to issue for his arrest.

IT IS FURTHER ORDERED that all remaining allegations are dismissed on motion of the United States. The defendant is advised of his right to appeal the imposed sentence by filing a Notice of Appeal within 10 days from the date of sentencing.
