COMES NOW PROBATION OFFICER Neil Graber presenting an official report on Rafael Ramirez
Urbina who was placed on probation November 22, 2004, by the Honorable Frederick J. l\/lartone
presiding in the District Court of Arizona. On November 16, 2005, probation was revoked and a four
month and seven day custody term was imposed, to be followed by a one year supervised release term.
Said supervision term commenced on the offenders discharge from imprisonment on December 14,
2005. In addition to the general terms and conditions adopted by the court in General Order 04-1 1, the
offender was ordered to comply with the following special conditions:
1. You shall participate as instructed by the probation officer in a program of substance abuse
treatment which may include testing for substance abuse. You shall contribute to the cost of
treatment in an amount to be determined by the probation officer.
2. You shall submit your person, property (including but not limited to computer, electronic devices,
and storage media), residence, office or vehicle to a search conducted by a probation officer, at
a reasonable time and in a reasonable manner.
3. You shall abstain from all use of alcohol or alcoholic beverages. l
Rafael Ramirez Urbina was convicted of false statement in connection with the acquisition of a firearm.
RESPECTFULLY PRESENTING PETITION FOR ACTION OF COURT FOR CAUSE
AS FOLLOWS:
Violation of Standard Condition 18: lf supervision follows a term of imprisonment, you shall report in
person to the Probation Office in the district to which you are released within seventy—two (72) hours of
release.
At his November 17, 2005, probation revocation disposition hearing, Urbina was advised by the Court
of the imposition of a one year term of supervised release to follow his custody term. Urbina was
released from custody on December 14, 2005, but has since failed to report in person to any United
States probation office. His whereabouts is unknown. A Grade C violation 7B1 .1(a)(3)(B).

AFFIDAVIT AND PETITION PRAYING THAT THE COURT WILL ORDER A WARRANT BE ISSUED
In conformance with the provision of 28 U.S.C. § 1746, I declare, under penalty of perjury, that the
foregoing is true and correct to the best of my knowledge. Based on the information presented that the
offender has violated conditions of supervision, I am petitioning the Court to issue a warrant.
