I initially saw Mr. Simms on Jimi: 25"‘. He gave a history of syrnptozns consistent with
rhinitis. . The symptoms started about Four moniss ago and consist of runny nose,
Sutffiness, postnassl drainage. and headaches. Occasionally he has tightness in the chest
or possible wheezing. Hee has also had some itching in the last few months. All the
symptom have been very bothersome.

Apparently almost all ofthe symptoms started only four months ago alter exposure to
smoke from 'ourning, plastic and fom:.
Family history is negative for atopy. . ‘ ‘
PHYSICAL EXAMINATION; Revealed slight edema and pallor of the turbinates; chest
clean heart rate eighty regularp no murmurs; there was no rash on the skin at this time.
Height 71 inches; weight 180 ppunds; blood pressure 118/68.
According to Zvi: Simms apprdxirnateiy four months ago the house next door burned to
the ground, his house also caught nre. His house was wrapped in a plastic material.
According to him the symptorits started after being exposed to this nre. He never had
any symptoms before. He gnoucd out of the home for about ten days and the Symptoms
cleared up, amost completely. However, when he renamed back to the house the

At the time of the surnrnary conference visit on July 2, 2001 instructions in
environmental control mcasurcslwere given.
Symptomatic tacrapy included samples ofN¤acert AQ, ence daily; Patanel, twice daily,
for itchy eyes. When I saw him'on July 2“° hc had scratchy throat and headaches.
