On the date of this Application, Judgment Debtors owe $214,920.27 plus interest

through April 10, 2007 in the amount of $33,909.08 on the judgment dated July 21, 2006, and $35,281.82 plus interest in the amount of $675.54 on the judgment dated November 22, 2006. The outstanding balance due on the Judgments as of April 10, 2007 is $284,786.71. 4. Applicant has made a demand on the Judgment Debtors for payment of the

amount adjudged due, but the Judgment Debtors have not paid that amount. Nor have the Judgment Debtors agreed and continued to pay the nonexempt portion of their wages until the judgment is satisfied. 5. The Garnishee is believed to be an employer of Judgment Debtor Teri Goodman

or otherwise owes or will owe to the Judgment Debtor Teri Goodman disposable earnings. 6. Applicant has not received notice of the Judgment Debtor Teri Goodman's intent

to enter into an agreement for debt scheduling with a qualified consumer credit counseling service or, if such notice has been received, that he timely objected, in writing, to the judgment debtor's participation in such an agreement or that he has been notified that the agreement is no longer effective.
