The motion at docket 83 is GRANTED CONTINGENT on the following: 1. The parties shall henceforth correct the case number to read CV-03-1915 PHX JWS. 2. On or before January 10, 2006, the parties shall file and transmit to the assigned judge's chambers ECF mailbox a revised version of the "Preliminary Order For Notice and Hearing In Connection With Settlement Proceedings" identical to that which was filed with the Stipulation and Settlement Agreement at docket 82, except that it shall include the following information: a. Page 1, l. 1 of the text insert "29" following "December" b. Page 1, ll. 9-10 of the text fill blanks so that the date reads "this 13th day of January 2006" c. Page 2, para. 5, date and time to read "April 7, 2006, at 10:30 AM" d. Page 4, para. 9, date to read "January 20, 2006" e. Page 6, para. 12(a,) date to read "March 20, 2006" f. Page 7, para. 13, date to read "March 20, 2006" g. Page 9 below para. 19, date to read "January 13, 2006" 3. On or before January 10, 2006, the parties shall file and transmit with the above document the following: (i) a revised "Notice of Pendency of Class Action . . . Fairness Hearing," (ii) a revised "Proof of Claim and Release", and (iii) a revised "Summary Notice Of Pendency . . . Settlement Hearing", each identical to that alread filed, except that the revised documents shall put dates in the blanks which correspond to the relevant dates set out in paragraph 2 of this order. ______________________
