Pursuant to an Order filed April 29, 2005, the Clerk of the Court sent plaintiff a packet including the documents necessary to effect service of the complaint. Plaintiff was instructed to

complete the forms and return them to the Clerk of the Court so that service could be made. Plaintiff has failed to return the

forms to the Clerk of the Court; thus, the complaint has not been served. Civil Local Rule 16.2(b)(2)(B)(i), Rules of Practice of the United States District Court for the District of Arizona provides that the maximum date to effect service is 60 days from the filing of the service order. Since plaintiff was provided the service

packet on April 29, 2005, the 60 days would have commenced on that date and has now run. IT IS THEREFORE ORDERED that plaintiff shall have twenty (20) days from the date of the filing of this Order to show good cause why this case should not be dismissed pursuant to Rule 4(m), Case 2:03-cv-02290-SRB Document 11 Filed 11/28/2005 Page 1 of 2

If plaintiff fails to
