COMES NOW PROBATION OFFICER R. Brian Colgan presenting an Official report on Angela
Renee FLORES who was committed to the Bureau of Prisons on October 25, 2004 by the Honorable
Neil V. Wake presiding in the District Court of Arizona. A 36 month period of supervised release was
imposed, and supervision commenced upon the offenders discharge from imprisonment on January 1 1,
2005.
On l\/lay 16, 2005, the Court revoked supervised release and ordered Flores serve 6 months in prison
and 30 months supervised release. On August 15, 2005, Flores was released form prison to begin her
term of supervised release.
in addition to the general terms and conditions adopted by the Court in General Order 04-11, the
offender was ordered to comply with the following special conditions:
1. You shall participate as instructed by the probation officer in a program of substance abuse
treatment which may include testing for substance abuse. You shall contribute to the cost of
treatment in an amount to be determined by the probation officer.
2. You shall submit your person, property (including but not limited to computer, electronic devices,
and storage media), residence, office, or vehicle to a search conducted by a probation officer, at
a reasonable time and in a reasonable manner.
3. You shall participate in a Home Confinement Program without electronic monitoring under the
sanction of incarceration for a period of 30 days.
4. You shall participate in a mental health program as directed by the probation officer which may
include taking prescribed medication. You shall contribute to the cost of treatment in an amount
to be determined by the probation officer.
Angela R. FLORES was convicted ot possession of stolen mail in violation of 18 USC § 1708 , a class

RESPECTFULLY PRESENTING PETITION FOR ACTION OF COURT FOR CAUSE
AS FOLLOWS:
A. Violation of Standard Condition No. 3: You shall report to the Probation Office as directed by
the Court or probation officer, and shall submit a truthful and complete written report within the
first five days of each month.
1. Flores failed to report as directed, in person for the month of January, on January 11,
2005, to the supervising probation officer in Phoenix, Arizona. In addition, she failed to
submit a written monthly supenrision report. A Grade C violation, §7B1.1(a)(3).
2. Flores failed to report as directed, in person for the month of February, by February 6,
2005 to the supervising probation officer in Phoenix, Arizona. In addition, she failed to
submit a written monthly supervision report. A Grade C violation, §7B1.1(a)(3).
B. Violation of Standard Condition No. 9: You shall not purchase, possess, use, distribute or
administer any narcotic or other controlled substance as defined in section 102 ofthe Controlled
Substances Act (21 U.S.C. § 801) or any paraphernalia related to such substances, without a
prescription by a licensed medical practitioner. Possession of controlled substances will result
in mandatory revocation of your term of supervision.
1. Flores consumed amphetamine andlor methamphetamine as evidenced bya positive urine
sample she provided on November 3, 2005. In addition, Flores signed a written admission
indicating she had used amphetamine and/or methamphetamine. A Grade C violation,
§7B1 .1(a)(3). ·
2. Flores consumed amphetamine and/or methamphetamine as evidenced by a positive urine
sample she provided on December 29, 2005. A Grade C violation, §7B‘1.1(a)(3).
C. Violation ofSpecial Condition No. 1:You shall participate as instructed bythe probation officer

1. Flores failed to submit a urine sample on December 19, 2005 at TASC in Phoenix, Arizona
as directed via the TASC "co|or line". A Grade C violation, §7l31.1(a)(3).
2. Flores failed to submit a urine sample on January 3, 2005 at TASC in Phoenix, Arizona as
directed via the TASC "color line". A Grade C violation, §7B1.1(a)(3).
3. Flores failed to submit a urine sample on January 19, 2005 at TASC in Phoenix, Arizona
as directed via the TASC "co|or line". A Grade C violation, §7B1.1(a)(3).
4. Flores failed to submit a urine sample on January 24, 2005 at TASC in Phoenix, Arizona
as directed via the TASC "color line". A Grade C violation, §7B1.1(a)(3).
