Petition to Revoke Supervised Release {/
COMES NOW PROBATION OFFICER Myra Roper presenting an official report on Mario White
who was committed to the Bureau of Prisons on January 10, 2005, by the Honorable Roger G. Strand
presiding in the District Court of Arizona. A three—year period of supervised release was imposed, and
supervision commenced upon the offenders discharge from imprisonment on September 7, 2007. In
addition to the general terms and conditions adopted by the court, the offender was ordered to comply
with the following special conditions:
1. You shall participate as instructed by the probation officer in a program of substance abuse
treatment which may include testing for substance abuse. You shall contribute to the cost of
treatment in an amount to be determined by the probation officer.
2. You shall submit your person, property (including but not limited to computer, electronic devices,
and storage media), residence, ofnce, or vehicle to a search conducted by a probation officer, at
a reasonable time and in a reasonable manner.
3. You shall participate in a mental health program as directed by the probation officer which may
include taking prescribed medication. You shall contribute to the cost of treatment in an amount
to be determined by the probation officer.
4. The defendant shall participate in an approved program for domestic violence.
5. You shall not contact the victims referenced in the offense conduct section of the presentence
report and the probation ofhcer will verify compliance.
6. You shall not be involved with gang activity, possess any gang paraphernalia or associate with _
any person affiliated with a gang.
7. You shall abstain from all use of alcohol or alcoholic beverages.
8. You shall cooperate in the collection of DNA as directed by the probation officer.
9. The defendant shall enroll in a General Education Development (GED) Program to obtain a high
school equivalency diploma.

RESPECTFULLY PRESENTING PETITION FOR ACTION OF COURT FOR CAUSE
AS FOLLOWS: .
A) Violation of Standard Condition No. 9: "You shall not purchase, possess, use, distribute or
administer any narcotic or other controlled substance or any paraphernalia related to such
substances, without a prescription by a licensed medical practitioner.”
On or about November 30, 2007, White used cocaine as evidenced by a urine test submitted that date
and his admission to USPO Roper. A Grade C violation.
On or about January 14, 2008, White used marijuana, cocaine, and methamphetamine as evidenced
by a urine test submitted that date and his signed admission. A Grade C violation.
On or about January 26, 2008, White used cocaine as evidenced by a urine test submitted that date.
A Grade C violation. .
" B) Violation of Special Condition No. 10: "You shall reside and participate in a residential re-entry
center for 120 days, unless discharged earlier by the probation officer."
On January 26, 2008, White absconded from Behavioral Systems Southwest (BSSW), Florence,
, Arizona, as evidenced by the incident report. A Grade C violation. -
AFFIDAVIT AND PETITION PRAYING THAT THE COURT WILL ORDER A WARRANT BE ISSUED
In conformance with the provision of 28 U.S.C. § 1746, I declare, under penalty of perjury, that the
foregoing is true and correct to the best of my knowledge. Based on the information presented that the
offender has violated conditions of supervision, I am petitioning the Court to issue a warrant.
