PETITION FOR ACTION ON CONDITIONS OF PRETRIAL RELEASE
COMES NOW DANIEL X. AGUILERA, PRETRIAL SERVICES OFFICER, of the Court
presenting an official report upon the conduct and attitude of releaseez Tatia Valenzuela, who
was released by the HONORABLE LAWRENCE O. ANDERSON sitting in the Court at
Phoenix, on the 25th day of May, 2006, who fixed bail at Personal Recognizance and imposed
the general terms and conditions of bail release adopted by the Court and also imposed special
conditions as follows: ‘
SEE ATTACHED ORDER SETTING CONDITIONS OF RELEASE
RESPECTFULLY PRESENTING PETITION FOR ACTION OF COURT FOR CAUSE
AS FOLLOWS:
ALLEGED VIOLATION:
1. The defendant shall remain and reside by virtue of placement at:
Behavioral Systems Southwest
2420 East Roosevelt
Phoenix, Arizona 85008
The defendant shall follow all program requirements including the direction of
staff members.
On November 1, 2006, Pretrial Services received a Notice of Intent to Terminate from
Behavioral Systems Southwest (BSSW). The attached letter reflects the defendant has
been noncompliant with BSSW program requirements.
