PETITION TO REVOKE CONDITIONS OF RELEASE
COMES NOW SUNNY R. CIELUCH, PRETRIAL SERVICES OFFICER, of the Court
presenting an official report upon the conduct and attitude of releasee: Tatia Valenzuela, who was
released by the Honorable Virginia A. Mathis sitting in the Court at Phoenix, on the @ day of
August, 2004, who fixed bail on the personal recognizance of the defendant and imposed the
general terms and conditions of bail release adopted by the Court and also imposed special
conditions as follows:

RESPECTFULLY PRESENTIN G PETITION FOR ACTION OF COURT FOR CAUSE AS O
FOLLOWS:
ALLEGED VIOLATION:
The defendant shall actively participate in any mental health treatment program as directed
by Pretrial Services. The defendant shall comply with all treatment requirements including
taking all medication as prescribed by his/her mental health care provider.
1. The defendant failed to report to mental health counseling as directed: December 13, 14, 20,
and 22, 2005; January 17 and 24, 2006; February l, 8, 21, and 27, 2006; and March 8 and 10,
2006.
The defendant shall submit to drug and/or alcohol treatment, not limited to urine drug testing
and breathalyzer tests, at the discretion of Pretrial Services.
2. The defendant failed to report for urinalysis testing as directed on February 28, 2006.

3. The defendant failed to report to Pretrial Services as directed for the month of _
February 2006 and as specifically directed by Ofiicer Smith on March 10, 2006. A

WARRANT BE ISSUED.
In conformance with the provisions of 28 U.S.C. § 1746, I declare under penalty of perjury,
that the foregoing is true and correct to the best of my knowledge. Based on the infomation
presented that the defendant has violated conditions of release, I am petitioning the Court to
