Subjective
IM in today due to 2wk hx of decr exercise tolerance. States that he had been doing 40 turns around the yard and
now he is not able to do any due to chest pain and difficulty breathing. States that he has no problems when at rest
but the problem starts with very little exhertion. States that the chest pain is like a burning in his chest and states that
it feels like when he had his heart attack. States that the pain lasts for as long as he continues to exhert himself and
abates shortly after beginning to rest. Also in today for f/u on abdominal mass noted on last CCC appt. Denies any

Mildly fatigued male no acute distress
ECG shows ST seg elevation in II, 111 and AVF which is also noted in ECG from Aug 05
Heart RRR
Lungs CTA
Abdomen BS present, LUQ mass noted, no tenderness or organomegaly noted.
Assessment
Probable unstable angina
LUQ abdominal mass.

Discussed with Dr. OSteen
send to MMC for eval unstable angina /abdominal mass
schedule to my line for eval in one week to fu on results of eval and schedule offsite consult pm
