county of MARICOPA, I am an Officer of the Court and I am qualified pursuant to RCP4 (d), 4 (e) and 45 (d), to
serve process in this case-
On December 6, 2005, I received a Subpoena Duces Tecum In a Civil Case; Notice of Deposition; and Letter dated
December 6, 2005 from attomey Richard Erickson of the law firm of Snell & Wilmer. In each instance I
personally served one copy each of the above listed documents, on those named below in the manner and at the
time and place shown, that all services except where noted were made within Maricopa County, Arizona.
I personally served Eugene Cole on December 6, 2005 at 1:57 pm, at his place of business, which is located at
2055 East Warner Road, Suite 101, Tempe, AZ 85284 Eugene H. Cole is described as male, approximately 50
years of age, of 6' height, 185# weight, and brown hair. I certify under the penalty of perjury the foregoing is
