Petition for Restoration to Capacity

TO THE HONORABLE JUDGE OF THE DISTRICT COURT: 1. 2. 3. 4. Petitioner, _______________________________, is interested in this matter as __________________________________________. A guardian, conservator, was appointed by order of this court dated ___________________________. The ward is no longer an incapacitated person and is able to provide for his/her own care. The protected person is no longer in need of a conservator and is able to manage his/her own property, or the property is no longer subject to waste or dissipation. The Court is requested to appoint an attorney to represent the abovereferenced ward protected person. The Court is requested to appoint a court visitor to report to the court regarding the above-referenced ward protected person.
