INSTRUCTION NO. _____ If you find in favor of Plaintiff on her claim of bad faith breach of insurance contract, then you shall consider whether the Plaintiff should recover punitive damages against the defendant. If you find beyond a reasonable doubt that the Defendant acted in a fraudulent, malicious, or willful and wanton manner, in causing Plaintiff's injuries, damages or losses, you shall determine the amount of punitive damages, if any, that the Plaintiff should recover.
