By signing below, the undersigned certifies that he/she is an active member in good standing of the State Bar indicated above. By submitting this form, the undersigned further agrees to the following: 1. I will adhere to the Court's procedures for the Electronic Case Filing System and follow all standards of practice and procedures set out in the Local Bankruptcy Rules, the Electronic Case Filing Participant Guide and the Administrative Procedures Guide [hereinafter collectively referred to as the "Rules and Guides"] for the Court. I will submit all pleadings, papers and orders to the Court electronically via the Electronic Case Filing System, except when otherwise permitted by the Rules and Guides. I will be assigned one (1) login name by the Court, and it will be my responsibility to protect the confidentiality of my login name and password. The login assigned to me will belong solely to me, and I am personally responsible for the use of the login and my password. If I allow other members of my firm or my staff to work in the CM / ECF system using my login, I will be responsible for any documents electronically submitted by them, and liable for any loss due to errors, omissions, damages, or misuse of the system. If I believe that my login or password has been compromised, I must change my password and notify the Court of the change immediately. I must also promptly inform the Court if I change my firm affiliation, business address, telephone number or e-mail address. The use of my login and password to electronically file any document via the Electronic Case Filing System, regardless of whether I personally file the document or authorize another person in my firm to file such document on my behalf, will constitute my signature upon that document for purposes of Fed. R. Bankr. P. 9011.
