Emergency Services Telephone Fee Return Instructions
These fees are in addition to other taxes and fees. The fees are each required on both local exchange service switched access lines and revenue-producing radio communication access lines as defined in Utah Code Ann. Section 69-2-5. Complete and file this return along with the required 911 supplemental schedule (TC-29Q) with the Utah State Tax Commission to the address on the front of the coupon by the last day of the month following the calendar month in which the fee is billed.

Enter the number of total taxable lines subject to the poison control fee. Apply the rate to the total lines and enter the amount. Adjustments may be made for bad debts and canceled services upon which the fee has been reported and paid in full by service providers to the Tax Commission. In the case of canceled services, a refund or credit for the fee must be given to the purchaser before the provider may take an adjustment. The adjustment amount allowed is the total emergency services telephone fees charged off as bad debt or canceled service, less any amounts recovered on the bad debts.

Enter the number of total taxable lines subject to the 911 emergency fee and identify them as radio and/or land. These totals must agree with the totals of the detail being submitted on the supplemental schedule (TC-29Q). Enter the dollar amount applicable to the total 911 fees. The seller discount on 911 fees is available for complete, timely-filed, paid returns. The discount should not be taken if the return is incomplete or if the return or payment is late. Calculate the seller discount applicable to the 911 fees and enter the amount.
