National Pollutant Discharge Elimination System for Concentrated Animal Feeding Operations

INSTRUCTIONS:

This sheet may be utilized to document information regarding manure distribution activities that is required to be maintained in the facility operating record. Facilities may choose to maintain and present the information in a different format. If an alternate format to present information is chosen, the format must be clearly organized and legible. This sheet is meant to serve as quick reference regarding the requirement to document manure distribution activities and to assist you in those activities. In the table below, list the date, recipient, their address, the type and amount of manure received or purchased by the recipient, and record that the required information sheet was provided to the recipient (a copy of the each information sheet must also be maintained in the operating record).

Manure Distribution Regulations: 327 IAC 15-15-17(a)(4) Requires that the operating record must contain, for a period of five (5) years, all applicable records of any person who receives or purchases more than ten (10) cubic yards or two thousand (2,000) gallons of manure, litter, or process wastewater in a year to include: A. the name and address of the person receiving or purchasing the manure, litter, or process wastewater; B. the amount of manure, litter, or process wastewater received or purchased by the person; C. a copy of the information sheet as required under 327 IAC 15-15-15 which provides: 1. the name and address of the CAFO providing the manure, 2. a statement indicating that it is unlawful to allow the manure, litter, and process wastewater to enter any waters of the state, 3. information on the nutrient content of the manure, litter, and process wastewater, based on the most current analysis, and 4. the manure, litter, and process wastewater application requirements of 327 IAC 15-15; and D. the date or dates of transfer.
