NOTE: BOTH PARTIES MUST FILE JUDICIAL COUNCIL FORM FL141 DECLARATION REGARDING SERVICE OF DECLARATION OF DISCLOSURE (PRELIMINARY) PRIOR TO SETTLEMENT CONFERENCE AND/OR TRIAL. This statement must be completed jointly between the parties. If the parties are unable to complete this statement jointly, mark this statement as a separate statement, provide the information required, including information for item 7. This worksheet must be submitted to the Family Law Clerk, 3055 Cleveland Avenue, Santa Rosa, California, 95403 ten (10) days before the settlement conference or trial, whichever is earlier. Please type or clearly print answers to all applicable questions on a separate paper and attach it to this coversheet. If this statement is a separate statement, a copy must be mailed to all parties and a proof of service must be filed with the original of this form. 1. State the following statistical information: Date of marriage or partnership and date of separation; Age and occupation of the petitioner and the respondent; The names and ages of minor children. 2. If child support, spousal support, partner support, or attorney's fees are in dispute, please do the following: a. For child support: i. ii. iii. iv. v. b.
