I have personally inspected and examined the merchandise described in the foregoing application and determine that the same contains excessive moisture, or other impurities, not usually found in or upon such or similar merchandise, as indicated above. This claim is based upon the following facts, if any, ascertained by me or under my direction.

We ask for the information in order to carry out the laws and regulations administered by the Customs and Border Protection. The form is used by United States importers to apply for a duty allowance due to damaged or defective imported merchandise and by CBP to authorize such a duty allowance. It is required to obtain or retain a benefit. The estimated average burden associated with this collection of information is 8 minutes per respondent depending on individual circumstances. Comments concerning the accuracy of this burden estimate and suggestions for reducing this burden should be directed to U.S. Customs and Border Protection, Information Services Branch, Washington, DC 20229, and to the Office of Management and Budget, Paperwork Reduction Project (1651-0007), Washington, DC 20503.
