The following plates will receive the same plate number as your current plate: Personalized, Collector, Collector Special, Hobbyist, and Amateur Radio. All other plate types will be issued a new number. Sesquicentennial replacement plates are no longer available. Customers needing to replace Sesquicentennial plates will receive a set of regular design plates. If you have personalized Sesquicentennial plates and wish to apply for regular personalized plates, send $15 personalized plate fee. Since personalized plates cannot be duplicated, your message may not be available. Please see the Special Plates Unit contact information on reverse side to check the availability of choices. Mail this completed and signed form with the correct fee. Do NOT send your title. Expect delivery in approximately 6-8 weeks. X

Send the application with the correct fee.
For Replacement License Plates (Nonpersonalized):
License plates with a new number will be mailed in 3-4 weeks.
Upon receipt of the new plates, destroy the old plate(s). If plates are stolen, notify your local law enforcement
agency of the stolen plate number.
Duplicate License Plates (Personalized): Note: Only issued for personalized plates AND the following special interest plates: Collector, CollectorSpecial, Hobbyist, Ameteur Radio You will be issued duplicate plate(s) with the same personalized number. Expect delivery in approximately 6-8 weeks. If your personalized license plates have been reported stolen, please be aware that when you receive the duplicate plates, you may be stopped by law enforcement in any state for possession of stolen plates.
