Use this form if you are a landlord, your tenant has created a health hazard on the property or has damaged the property, and you want to give the tenant notice to either correct the problem in 7 days or move out.

If the tenant does not remove the health hazard, repair the damage, or move out as requested in the Notice to Quit, you can file a complaint (Form DC 102b) with the district court requesting the court to order eviction.
