Talent and Model Releases are used when giving permission for your photograph or likeness to be used in connection with advertising or other promotions. A parent or guardian must sign this type of release for a minor child.

This Talent and Model Release is entered into on _____________, between ______________________________ (Name & Address) herein referred to as ("Releaser"), and _______________________ (Name & Address), herein referred to as ("Manufacturer").
FOR THE GOOD AND VALUABLE CONSIDERATION, the receipt and sufficiency of which are hereby acknowledged:
Releaser hereby give, consent, and forever grant to Manufacturer, its representatives, licensees, marketers, and any other related parties or publishers of its promotional materials and their successors and assigns, the right to use, publish and copyright Releaser’s picture, portrait or likeness, in whole or part, including alterations, modifications, derivations, and composite thereof, in advertising and promotions throughout the world.
This right shall include the right to combine Releaser’s likeness with others and to alter my likeness by digital or other means.
Releaser acknowledges his or her full and complete satisfaction with the terms of this Talent and Model Release.
