sworn, deposes and states as follows:
I, as a Contractor under certain contract dated
_____________________ with _____________________ (Client) have
furnished labor, services and materials on the following premises
_____________________ (the "Premises").
For good and valuable consideration, the receipt and
sufficiency of which is hereby acknowledged, I hereby waive, discharge, and
releases any and all liens, claims, and rights to liens against the above
mentioned project, and any and all other property owned by or the title to
which is in the name of the Client and against any and all funds of the Client
appropriated or available for the said project, and any and all warrants drawn
upon or issued against any such funds or monies, which I may have or may
hereafter acquire or possess as a result of the furnishing of labor, materials,
and Services by me on or in connection with said project, whether under and
pursuant to the above-mentioned contract
between me and Client pertaining to said project or otherwise, and which said
liens, claims or rights of lien may arise and exist.
I hereby further acknowledge that the sum of
_____________________ Dollars constitutes the entire amount connection with
said project and that the payment of said amount will constitute payment in
full and will fully satisfy any and all liens, claims, and demands which I may
have against the Client in connection with said contract.

Description
After a contractor has performed certain work for a client and has been paid in full he will generally provide a waiver and release off all liens and claims. This shows that the there are no outstanding amounts due and that everything is squared away. The form contains information about the contractor, client, the premises and more.
